Short Term Disability

Eligibility Employees who have worked at least four consecutive weeks.
 
Effective Date First of the month following 30 days of employment.
 
Basic Benefit 50% of your average weekly wage up to a maximum of $170.00 a week.
 
Waiting Period Seven calendar days of disability due to off the job illness or injury for which no benefits are payable.  Benefits rights begin on the eighth consecutive day of disability.
 
Definition of Disability Employee is prevented from performing work because of off the job illness or injury.  Disability connected with pregnancy is included.
 
Benefit Duration Benefits are paid for a maximum of 26 weeks during 52 consecutive weeks or during any one period of disability.
 
Time Limit for Filing a Claim 30 days after the first day of disability
