Short Term Disability |

| Eligibility | Employees who have worked at least four consecutive weeks. |
| Effective Date | First of the month following 30 days of employment. |
| Basic Benefit | 50% of your average weekly wage up to a maximum of $170.00 a week. |
| Waiting Period | Seven calendar days of disability due to off the job illness or injury for which no benefits are payable. Benefits rights begin on the eighth consecutive day of disability. |
| Definition of Disability | Employee is prevented from performing work because of off the job illness or injury. Disability connected with pregnancy is included. |
| Benefit Duration | Benefits are paid for a maximum of 26 weeks during 52 consecutive weeks or during any one period of disability. |
| Time Limit for Filing a Claim | 30 days after the first day of disability |
